It is not a standard OFA film. I can only say the hip is luxated. I suspect it is traumatic. Of course. the hip joints may have CHD in the beginning. If the dog is painful, the surgery is a better choice. I didn't see the dog in person. So, I cann't say whether the surgery is necessary for sure. It hard to say what kind of price is reasonable. It depends on what you expect and what they offer. At least find a vet who can take the hip xray and put the film with the right way.
